- $50 per patient per month, and only for the patient's first 12 months

You earn fifteen percent (15%) of the Net Subscription Revenue we collect from each Active Referred Patient, every month they remain active, for the first twelve (12) months of their subscription, SUBJECT TO A FIFTY US DOLLAR ($50) PER-PATIENT-PER-MONTH CAP.
